Referring Physician FAQ

How do I refer a patient?

Please fax referrals to 416-742-0076 or email us (please do not include patient demographics in your email) for more information. Please include patient name, DOB, contact number, clinical issue or preferred procedure (if known), and any relevant medical history. Please include your OHIP billing number.

What services require a referral?

A referral is required for non-cosmetic vascular surgery and plastic surgery consultations, including:

  • Aneurysms, carotid disease, peripheral vascular disease, lower extremity wounds

  • Skin cancer excision

  • Trigger finger or carpal tunnel assessment

  • Endometrial biopsies

Can patients self-refer for services?
Yes. Patients may book directly for:

  • Medical and Cosmetic Botox

  • PRP treatments

  • Skin Laser treatments

  • IUD and Nexplanon insertion

  • Pap testing

  • Metabolic optimization

  • Hormone and menopause care

Will my patient be followed long-term?
We provide procedure-based care and return patients to their referring provider with a consultation summary and treatment report.

Where are procedures performed?
Minor procedures are done in our clinic. Surgeries requiring general anesthesia are performed at Humber River Hospital.
