Referring Physician FAQ

How do I refer a patient?

Please fax referrals to 416-742-0076 or email us (please do not include patient demographics in your email) for more information. Please include patient name, DOB, contact number, clinical issue or preferred procedure (if known), and any relevant medical history. Please include your OHIP billing number.

What services require a referral?

A referral is required for non-cosmetic vascular surgery and plastic surgery consultations, including:

  • Varicose veins,

  • Skin cancer excision

  • Cyst or lipoma removal

  • Scar revision

  • Trigger finger or carpal tunnel assessment

Can patients self-refer for aesthetic or contraceptive services?
Yes. Patients may book directly for:

  • Botox and filler

  • PRP treatments

  • Laser and skincare services

  • IUD and Nexplanon insertion (though referrals are welcome)

Will my patient be followed long-term?
We provide procedure-based care and return patients to their referring provider with a consultation summary and treatment report.

Where are procedures performed?
Minor procedures are done in our clinic. Surgeries needing general anesthesia are performed at Humber River Hospital.

Do you require pre-referral imaging or labs?
Not always. If additional diagnostics are required, we’ll coordinate after the consult unless provided in your referral.
